Our Facility
Trailhead Treatment Center is located at 103 Stiles Rd, Suites 1 & 2, Salem, NH 03079. The facility was designed to feel warm, personal, and inviting. Natural lighting, abundant plant life, and warm color schemes throughout create an environment built for healing, not one that feels clinical or institutional.

Group Therapy Rooms
Group sessions are the backbone of PHP and IOP programming. Trailhead's group rooms are designed for comfort during sessions that can run several hours. Morning group therapy, psychoeducation, relapse prevention, and processing sessions all take place here.

Individual Therapy Offices
Each client is assigned an individual therapist on day one. That therapist stays with you through your entire course of treatment. Individual sessions happen in private offices designed for confidential, focused clinical work.

Quiet Room and Holistic Programming Space
A dedicated quiet space is used for yoga, meditation, breathwork, sound therapy, art therapy, ear acupuncture, Reiki, and grounding techniques. This is where the holistic programming that Co-Founder Caitlin DiBlasi shaped from her background as a yoga instructor takes place. Holistic sessions are scheduled into every treatment day alongside clinical work.

Cafeteria
Lunch is provided on-site for PHP clients. The cafeteria provides a break between morning clinical sessions and afternoon programming.

Outdoor Spaces
Trailhead incorporates nature-based programming and outdoor spaces into the treatment experience. Salem sits in southern New Hampshire, where natural surroundings support the grounding and nature-integrated approach built into the program.

Trailhead's facility was intentionally designed to avoid the clinical, institutional feel of many treatment centers. The environment reflects the philosophy behind everything at Trailhead: personal, warm, and built by people who know what it feels like to walk through those doors for the first time.

Salem is located on the I-93 corridor at the Massachusetts border, accessible from both the Boston metro area and communities throughout southern New Hampshire. The facility serves residents of Salem, Windham, Plaistow, Londonderry, Derry, Pelham, and Massachusetts border communities including Haverhill, Methuen, and Lawrence.
Parking is available on-site. No facility-provided transportation at this time.

All programs operate from our Salem location:
- Partial Hospitalization Program (PHP)— 9:00 AM to 3:30 PM, Monday through Friday
- Intensive Outpatient Program (IOP)— 9:00 AM to 12:30 PM, evening scheduling coming soon
- Outpatient Program— Scheduled individual sessions
- Medication-Assisted Treatment (MAT)— Vivitrol, Sublocade, Brixadi
- Mental Health Treatment— Dual licensed for SUD and mental health
- Family Support Services— Therapist-led family sessions
Women-only track available within both PHP and IOP.

Yes. Trailhead is located on the ground floor with accessible entry and on-site parking outside the building.
Yes. Lunch is provided on-site for PHP clients. The cafeteria provides a break between morning clinical sessions and afternoon programming.
PHP runs 9:00 AM to 3:30 PM, Monday through Friday. IOP runs 9:00 AM to 12:30 PM. Evening IOP scheduling is coming soon. Outpatient and MAT appointments are scheduled individually.
Smoking and vaping are permitted only in designated outdoor areas during scheduled breaks. Not allowed inside the building.
